英語の紹介状 Ver.2（つよぽん医学講座その４-２）

アメリカ人の骨折患者の帰国に際し英語で紹介状を書いて、
日系アメリカ人医師の友人に添削してもらった話を昨日書きました。
昨日のエントリーから添削後の文だけ、も一度掲載します↓

To whom it may concern,
　
Mr.Andy is a ○○ year old male, who fell down and had injured bimalleoller fracture of the right ankle on Jan ○○, 2009. Open reduction and internal fixation (ORIF) was done on the same day. Medial malleolus (tibia) was fixed with K-wires and suture wire (tension band wiring), and lateral malleolus (fibula) was fixed with 1/3 tubular plate of SYNTHES. Good stability and alignment of the fracture site were accomplished after the operation. Please check the copy of the enclosed operation report and X-ray film on which the details are shown. Below the knee cast was applied. One week post operatively, all stitches were removed. Casting for several weeks and range of motion exercise and partial to full weight baring gait exercise after taking off the cast are recommended.

Mr.Andy is a ○○ year old asshole, who got drunk and fell down, and as a result experienced a bimalleoller fracture of the right ankle on Jan ○○, 2009. Open reduction and internal fixation (ORIF) was performed on the same day without general anesthesia or septic technique because he did not have private insurance. Medial malleolus (tibia) was glued with K-wires and suture wire (tension band wiring), and lateral malleolus (fibula) was fixed with 1/3 tubular plate of SYNTHES to his other leg. Good stability and alignment of the fracture site were accomplished after the operation. For whatever reason we never understood, he was in a lot of pain during the surgery. Also, because he will not be able to walk for a while(if ever for that matter), we had not only left a Foley catheter, but also sutured it tight to his penis, and on top of that, we liberally applied Dermabond for extra security. Please liberate his urethra when indicated. Please check the copy of the enclosed operation report and X-ray film on which the details are shown. Below the knee cast was applied at the end of the procedure. One week post operatively, all stitches were removed, and the wound looks massively infected. Casting for several weeks and range of motion exercise, and partial to full weight baring gait exercise after taking off the cast are recommended, and also, if the infection does not improve, please proceed to amputate. Perhaps he should undergo general anesthesia this time.
　
Being an American, I am not so sure if Mr.Andy is intelligent enough to seek medical attention at all, and that even if he did he still has a very good chance that he will receive sub-optimal care due to general lack of competency in American medical community in general.
